Forms - Land Titles


By clicking on the forms below, you will be able to retrieve a copy of the form complete with the most common associated affidavits that may be necessary to be completed to make the document registrable. For additional related affidavits that may be required, please see the Supplemental Forms at the end of page 2 below. Each form also contains a link to the applicable section(s) of the Land Titles Manual in order to assist you in completing the forms. Please note that forms may be filled in on-line, saved, and then printed for manual submission of original forms to the Land Titles Office.

Documents submitted for registration must be accompanied by an electronic on-line Document Registration Request (DRR) form. To create a DRR, logon to our Spatial Information (SPIN 2) system, http://www.spin.gov.ab.ca, click the DRR button on the SPIN 2 Menu Bar then select Create DRR; see Create DRR help file for additional information. 

**Note:  When printing forms from this manual, turn off the 2 sided printing option on the Adobe X print screen when it comes up by de-selecting "print on both sides of paper".

Supplemental Forms:

  • Affidavit of Execution (pdf) - for use when additional affidavits required - remember to attach the original copy of the affidavit to the original document.
  • Affidavit of Minors for Estates (pdf) - for use when completing a transfer, mortgage or other instrument executed by an executor, administrator or trustee under a will except an application for transmission or a caveat or a discharge of mortgage - remember to attach the original copy of the affidavit to the original document.
  • Affidavit of Officer (pdf) - for use when dealing with acquisitions or dispositions of real property by an incorporated congregation executed by an Officer of the congregation (must be completed by an Officer other than the Officers signing the acquisition or dispostion) - remember to attach the original copy of the affidavit to the original document.
  • Affidavit Re Value of Land (pdf) - for use when registering Orders that cancel a title or when affidavit is completed in another office - remember to attach the original copy of the affidavit to the original document.
  • Affidavit Verifying Corporate Signing Authority (pdf) - for use when an Officer or Director is signing a document on behalf of a Corporation that is not affixing a Corporate Seal - remember to attach the original copy of the affidavit to the original document.
  • Attached Schedule (pdf) - for use when information exceeds a document's data entry field, please note that all schedules must be clearly named or identified - remember to attach the original copy of the schedule to the original document.
  • Dower Affidavit (pdf) - for use when one person is signing some documents - remember to attach the original copy of the schedule to the original document.
