Available Stillbirth Documents

Four kinds of stillbirth documents are available to order from the Government of Alberta.

Photocopy of a Registration of Stillbirth

  • This document is a photocopy of a form completed at the time of stillbirth, usually by the mother.
  • Includes:
    • Full name of the stillborn child
    • Date of stillbirth
    • Place of Stillbirth
    • Sex of the child
    • Mother's name 
    • Mother's place of birth
    • Father/co-parent's name
    • Father/co-parent's place of birth
    • Registration number 
    • Registration date
  • Photocopies of Registrations of Stillbirth are generally only needed for genealogical purposes.
  • There is always an official raised seal and red stamp on the back of the photocopy stating it is a certified true copy of the original registration.

Certificate of Registration

  • As official certificates are not produced for stillbirths, Vital Statistics offers a Certificate of Registration. This is not an official government document; the photocopy of the Registration of Stillbirth serves this purpose.
  • The Certificate of Registration is a remembrance document for things like a memory book.
  • Includes:
    • Full name of the stillborn child
    • Date of stillbirth
    • Place of Stillbirth
    • Sex of the child
    • Mother's name 
    • Father/co-parent's name
    • Registration number 
    • Registration date
  • The Certificate of Registration is issued with a photocopy of a Registration of Stillbirth.

Photocopy of a Medical Certificate of Stillbirth

  • This document is a photocopy of a form completed at the time of stillbirth by the attending physician or medical examiner.
  • The information on this document describes the cause of stillbirth.
  • Includes:
    • Full name of the stillborn child
    • Date of stillbirth
    • Place of stillbirth
    • Sex of the child
    • Medical cause of stillbirth
    • Name of attending physician or medical examiner
    • Registration number
  • There is always an official raised seal and red stamp on the back of the photocopy stating it is a certified true copy of the original registration.

Stillbirth Record Search

  • A stillbirth record search will tell you if a record was found, but will not provide any other information.
  • Anyone may apply for a record search letter.
