Photocopy of a Registration of Stillbirth 

  • This is a copy of the registration completed at the time of event, usually by the parents

  • The photocopy print size is 21.5 X 28 cm (8 1/2 X 11 in.) (there is always an official raised seal and red stamp on the back stating it is a certified true copy of the original registration)

  • The information contained on the photocopy depends upon the year the event occurred as Vital Statistics has changed the form and the data it collects over the years

    Note: It will however always contain the following information, quite often more:

    • Full name of the child
    • Sex of the child (male or female)
    • Date of stillbirth
    • Place of stillbirth
    • Parents names and their birthplaces
    • Registration number
    • Registration date

Medical Certificate of Stillbirth 

  • This is a photocopy of the Medical Certificate of Stillbirth completed at the time of the event, usually by the attending physician

  • The photocopy print size is 21.5 X 28 cm (8 1/2 X 11 in.) (there is always an official raised seal and red stamp on the back stating it is a certified true copy of the original registration)

  • The information contained on the photocopy depends upon the year the event occurred as Vital Statistics has changed the form and the data it collects over the years

    Note: It will however always contain the following information, quite often more:

    • Full name of the child
    • Sex of the child (male or female)
    • Date of stillbirth
    • Place of stillbirth
    • Medical cause of stillbirth
    • Name of attending physician or medical examiner
    • Registration number
